Answer
Many scholars prohibit the use of mouthwash with alcohol that intoxicates, especially if derived from grapes or dates, due to it being najas (impure).
The fatwa states: it is not allowed to use medicine mixed with intoxicants to rinse and clean the mouth and teeth or for disinfection due to the presence of permissible and available substitutes.
Some scholars permit the use of such mouthwash if the alcohol is synthetic, non-intoxicating, and used for medicinal purpose, provided it is not ingested. Scholar Shaykh Sa'd Al-Humayd mentions: "If the alcohol which this mouthwash contains is not intoxicating, then it is ok to use it, so long as it is not harmful."
As a precaution, scholars often recommend using alcohol-free mouthwashes or natural alternatives like saltwater.
